Requesting Placement for Graduate Nursing Clinical Rotations
Requirements for Placement

Student must be a full-time or part-time Prisma Health team member with minimum 1-year of employment OR be a Clemson University or University of South Carolina Graduate Nursing student.
Student must attend a current Prisma Health affiliated School of Nursing.
No new affiliation agreements will be considered unless there is a specific workforce need. If you are enrolled in a non-affiliated program please contact NursingAcademics@prismahealth.org for further assistance.
Student must hold a valid license as a Registered Nurse (RN) as defined by the State of South Carolina Board of Nursing.
Student must have professional liability insurance obtained personally or through School of Nursing.
The “Independent Student Request” should not be submitted until the student has secured a Prisma Health preceptor.
Student is responsible for securing their own Prisma Health preceptor.
A preceptor verification form must be completed and uploaded to the documents tab in their myClinicalExchnage compliance checklist.
Approved preceptors for Nurse Practitioner programs include Prisma Health nurse practitioners (must have held their APRN license for a minimum of 2 years and have approval from their supervisor to serve as a preceptor), physicians, or physician assistants (for pediatrics only).
Approved preceptors for Midwifery programs include Prisma Health midwives, nurse practitioners (must have held their APRN license for a minimum of 2 years and have approval from their supervisor to serve as a preceptor), or physicians.
The Independent Student Request must be submitted in a timely manner. A student may not begin a clinical placement the same week they submit a placement request. Please allow a minimum of 2 weeks to complete the required Prisma Health student clearance process once your request form has been approved by Nursing Academics.
The Independent Student Request must be filled out completely. Incomplete forms will be returned to the student.
