Clearance Items
To complete the clearance requirements, navigate to the My Rotations section in mCE. There will be a red "pending" link next to any areas that require your attention. View detailed instructions for completing the clearance requirements below.
You may only begin your Prisma Health instructor, staff or scholar role once you have submitted your clearance documents and they have been approved by Student Affairs Administration.
Common Requirements
The following items are required for all instructors, staff and scholars.
If you are also a Prisma Health employee, please email Student.Affairs@PrismaHealth.org for options on reducing your required compliance items.
What’s required:
- A background check report must be conducted on all Prisma Health student learners, instructors, staff and scholars 18 years of age and older.
- Background check reports must be dated no more than 12 months prior to the initial Prisma Health educational experience start date. This report is valid for all future rotations at Prisma Health.
- Background check reports must contain the following information:
- Social Security verification
- State criminal history search
- Sexual Offender Registry
- Officer of Inspection General
- System for Award Management (previously known as Excluded Parties List System – EPLS)
- Patriot Act
- All positive background reports must be submitted directly to your designated Student Affairs Administration representative for approval by the Student Affairs Administration executive director. If denied, individuals may have the ability to appeal such decisions directly to Prisma Health.
- A background check can be acquired in two ways:
- Your college/university: If your school has completed a background check for you during your enrollment and it meets the guidelines above, please obtain a copy of your background check report to upload into mCE.
- A background check company: We will accept any background check company as long as it reports on all areas listed in the above policy.
- For those seeking a company to conduct a background check report, we work closely with SR&I.
- Go to the SR&I website: www.srandi.com.
- Click on the “My Background Check” link at the top.
- Click on the large “Affiliate” button in the middle of the page.
- Enter the following Affiliate Program Code: P68LB2
- For Prisma Health EMT or MedEx Academy students: Contact your coordinator or Student Affairs Administration for the appropriate code
- Click the blue “Next” button to complete the information requested on the form. In order for your background check to automatically sync to mCE, you must use the same email address and name that is associated with your mCE account.
- Submit payment.
- Create an account so that you can access your background check report once it has been completed.
- After you have received an email notification that it has been completed, please log in to SR&I to download your report and upload it to your mCE Compliance Checklist.

How to complete the compliance checklist:
- Attach your completed background check report and select "Documentation meets requirements" from the new value dropdown.
Your photo will be used for your Prisma Health identification badge. The photo must be saved in JPEG format and follow the same requirements as a passport photo:
- Plain blue/gray/neutral background
- No sunglasses, hats or headphones
- Full face directly facing the camera
- Natural facial expression
DO NOT use a picture of your ID or passport. These often have a hologram and do not print clearly. Selfies are acceptable as long as they follow the same requirements as listed above.
Note: If for any reason Prisma Health Badge Security Office decides the photograph uploaded is not acceptable, you will have to take a new photograph upon arrival to their department. Uploading a clear photo in mCE will save you time in the long run.
How to complete the compliance checklist:
- Upload your photograph in the “Supporting Documents” section in jpeg format only (not PDF or png).
- Select "Photo meets Badge ID requirements" in the new value dropdown.
- Enter your DOB (mm/dd/yyyy) in the “New Value” box.
- DO NOT upload any documentation of your DOB in the “Supporting Documents” section.
- Enter the name of the college/university for which you work or attend in the “New Value” box.
- DO NOT upload any documentation in the “Supporting Documents” section.
- If you are a Prisma Health team member, enter your 7-digit Employee ID number in the “new value” box. This number can be found in two places:
- In your Workday profile under "Job"
- In your team member Learning Hub account under User Record as your "User ID"
- Instructors, Staff and Scholars who are NOT employed by Prisma Health should enter “N/A” in the “new value” box.
- Enter your (full) SSN in the “New Value” box.
- If you do not have a SSN, please use your home country’s identification number or a combination of other numbers you can remember.
- No sequential numbers (e.g., 123-45-6789 or 456-78-9101)
- No numbers that are all the same (e.g., 222-22-2222)
- May not enter all zeroes (i.e., 000-00-0000)
- If you do not have a SSN, please use your home country’s identification number or a combination of other numbers you can remember.
- DO NOT upload any documentation of your SSN in the “Supporting Documents” section.
Learning Hub is a learning management system (used by all Prisma Health employees, contract workers, students and volunteers) that provides you with important healthcare modules required to begin your instructor, staff or scholar role (HIPAA, infection control, patient rights, etc.). All assigned modules must be completed prior to your rotation start date. Due dates listed in Learning Hub are not relevant for students, instructors, staff or scholars.
Instructors, Staff and Scholars on Campus
The following items are required for any person who will be working in a Prisma Health facility.
TB (Tuberculosis) Screening:
First Time Students, Faculty, Staff and Scholars (one of the following upon initial clearance):
- QuantiFERON® or T-spot blood tests, drawn within the past year.
- A positive test indicates TB infection (either active or latent). Chest X-ray and further evaluation will be necessary. Please see the section below on Positive Screenings.
- If you have received the BCG vaccine, a PPD will likely give a positive result. It is recommended that you obtain a blood test for the most accurate screening results and to prevent the necessity of undergoing additional testing.
- Two-Step TB PPD test (this is a 4 step process - 2 placements and 2 readings), completed within the past year. The following is the standard process for completing a two-step TB PPD test:
- Visit 1, Day 1: PPD antigen is applied under the skin.
- Visit 2, Day 3: PPD test is read (within 48-72 hrs. of placement). If positive, it indicates TB infection and a chest x-ray and further evaluation is necessary.
- Visit 3, Day 7-21: a second PPD skin test is applied (for those that test one was negative)
- Visit 4, 48-72 hours after placement: the second test is read.
- A positive 2nd test indicates TB infection in the distant past. Chest X-ray and further evaluation will likely be necessary. Please see the section below on Positive Screenings.
- Annual TB PPD test: An annual test will only be accepted if you have previously received a two-step TB test and have maintained annual 1-step testing each following year. Documentation of the two-step test plus all annual tests since then are mandatory.
- Chest X-ray: Only accepted when a person has a positive TB test. The chest X-ray must be done within 6 months prior to the start of the rotation.
- A positive test indicates active TB infection. Please see the section below on Positive Screenings.
Positive Screenings
- If you receive a positive blood or PPD screening, you are required to obtain a chest X-ray to determine if you have active Tuberculosis disease.
- Negative Chest X-ray: you will likely be offered optional treatment for latent Tuberculosis disease.
- If you accept the treatment plan, please email all related documentation to Student.Affairs@PrismaHealth.org.
- Student Affairs will reach out to confirm you completed the treatment, but no further action will be required for future rotations.
- If you decline treatment, you will need to email all TB documentation along with a TB Symptom Questionnaire to Student.Affairs@PrismaHealth.org.
- Symptom questionnaires will be required for subsequent rotations on an annual basis.
- If you accept the treatment plan, please email all related documentation to Student.Affairs@PrismaHealth.org.
- Positive Chest X-ray: you will be required to report to DHEC and undergo treatment for active Tuberculosis infection.
- Please email all related documentation to Student.Affairs@PrismaHealth.org. Prisma Health Employee Health will confirm all documentation and provide a date in which you are eligible to start your rotation.
- Student Affairs will reach out to confirm you completed the scheduled treatment, but no further action will be required for future rotations.
- Negative Chest X-ray: you will likely be offered optional treatment for latent Tuberculosis disease.
Returning Students, Faculty, Staff and Scholars:
- Prisma Health does not require annual TB screening for students, faculty, staff and scholars in subsequent rotations unless there is a break between rotations of over one year. In this case, you must obtain initial testing as described above (2-step PPD or TB blood test).
Note: TB PPD (skin) tests cannot be administered within 30 days of MMR or Varicella vaccine; live vaccines may cause a false positive for the TB PPD (skin) test.
How to complete the compliance checklist:
- Upload all TB documentation from your health care provider stating the date(s) and result(s) of the test(s) to mCE.
- PPD documentation must show the placed and read dates along with the results of both tests.
- Select the appropriate statement from the new value dropdown:
- Test(s) were negative for TB
- Test(s) were positive for TB, but negative chest Xray
- Test(s) were positive for TB, and received TB treatment
- This option should be selected for those who obtained both mandatory and optional treatment
What’s required:
- Documentation of two vaccinations or positive titers.
- In cases of partial negative titers:
- If only the Rubella is negative, you need to receive one MMR vaccine.
- If either the Measles (Rubeola) or Mumps titer is negative, you must receive two MMR vaccines.
- Equivocal titer results are considered negative.
- Please note: MMR vaccines are given 30 days apart.
How to complete the compliance checklist:
- Upload documentation from your health care provider stating the dates of your two vaccinations or positive titer results. Make sure your name is clearly visible on the documentation.
- Select "Documentation meets requirements" in the new value dropdown.
What’s required:
- Documentation from your health care provider stating the date of your CURRENT SEASON flu vaccination. Make sure your name is clearly visible on the documentation.
- You may request a medical or religious exemption to the influenza vaccination:
- To complete a medical exemption request, email the completed Influenza Vaccine Medical Exemption Request form to Student.Affairs@PrismaHealth.org. Student Affairs will submit your form to Employee Health for their review on your behalf.
- To complete a religious exemption request, complete the Religious Exemption Request form, and upload it to your myClinicalExchange (mCE) compliance checklist.
- Please note, if you are a Prisma Health Team Member, you will upload your approved Team Member exemption to mCE. Please do not submit a second exemption request for your student and/or instructor role.
How to complete the compliance checklist:
- If your rotation starts between November 1–March 30, you must receive the current season’s flu vaccination to start your rotation or submit exemption documentation as outlined below.
- Vaccinations
- Upload documentation of your fall 2022 flu vaccination.
- Enter the renewal date of NEXT flu season (11/01/2023) in the “New Value” box of mCE.
- Approved Medical Exemption
- Upload the approved exemption request form after it has been completed by Employee Health.
- Enter the renewal date of NEXT flu season (11/01/2023) in the “New Value” box of mCE.
- Religious Exemption
- Upload the completed religious exemption request form. Please note, if your documentation does not include all requirements, it will be declined.
- Enter the renewal date of NEXT flu season (11/01/2023) in the “New Value” box of mCE.
- Vaccinations
- If your rotation starts outside of flu season (April 1–October 31) but will run through flu season (past November 1), you will need to complete this item in order to be cleared.
- Upload a placeholder document (blank document or your immunization record).
- Enter the renewal date (11/1/23) in the “New Value” box of mCE.
- Reminder: You MUST upload your fall 2022 flu vaccination before November 1, 2022.
- If your rotation occurs completely outside of flu season (April 1–October 31), you may leave this item blank.
What’s required:
Hep-B is recommended but not required.
- If you decide not to waive the Hep-B vaccination, submit one of the following:
- Documentation of a complete vaccination series
- 3-series Engerix or Recombivax vaccinations
- 2-series Dynavax Heplisav vaccinations (documentation must show brand name to be approved with only 2 vaccinations on record)
- Documentation of a positive titer
- Equivocal titer results are considered negative.
- Titers should be drawn no sooner than 6 weeks post vaccination.
- Documentation of a complete vaccination series
- If you choose to waive your Hep-B vaccination, sign and date the waiver.
How to complete the compliance checklist:
- Upload documentation from your health care provider stating the dates of your three vaccinations or positive titer results. Make sure your name is clearly visible on the documentation.
- For those who sign the waiver, upload it as an attachment in the mCE compliance checklist under Hep-B.
- Select "Documentation meets requirements" in the new value dropdown.
What’s required:
- Documentation of two vaccinations or a positive titer.
- Equivocal titer results are considered negative.
How to complete the compliance checklist:
- Upload documentation from your health care provider stating the dates of your two vaccinations or your positive titer results. Make sure your name is clearly visible on the documentation.
- Select "Documentation meets requirements" in the new value dropdown.
Note: Prisma Health does NOT accept “history of having the chickenpox” as proof of immunity. You must have either two varicella vaccinations or a positive titer.
What's required:
- Select your COVID-19 vaccination status in the dropdown box.
- Select “Fully Vaccinated” if you have received a complete dose of the COVID-19 vaccine (2 vaccinations for Moderna and Pfizer, 1 vaccination for J&J/Janssen).
- Select “Medical Exemption” if you have a Prisma Health-approved medical exemption from receiving the COVID-19 vaccine.
- Select “Religious Exemption” if you have a Prisma Health-approved religious exemption from receiving the COVID-19 vaccine.
- Please note, the status chosen in this item must match the documentation uploaded in the COVID Vaccination/Exemption Documentation item below.
What's Required:
- A complete dose of COVID-19 vaccine (2-series Pfizer or Moderna, 1 vaccine Jannsen/J&J)
- You may request a medical or religious exemption to the COVID-19 vaccination:
- To request a medical exemption, email the completed COVID Vaccine Medical Exemption Request form to Student.Affairs@PrismaHealth.org. Student Affairs will submit your form to Employee Health for their review on your behalf.
- To request a religious exemption, complete the Religious Exemption Request form, and upload it to your myClinicalExchange (mCE) compliance checklist.

How to complete the compliance checklist:
- For those who answered "Fully Vaccinated” or "Fully Vaccinated + 1 (or 2) booster(s)" in the COVID-19 Vaccination Status section, enter the date you received your most recent COVID-19 vaccination in the New Value box, and upload your vaccination documentation.
- Please note, documentation for this item must match the status selected in the "COVID-19 Vaccination Status" item above.
- For those who answered “Medical Exemption” in the COVID-19 Vaccination Status section, enter the date your request was approved by Employee Health in the New Value box, and submit the approved exemption document.
- For those who answered “Religious Exemption” in the COVID-19 Vaccination Status section, enter today's date in the New Value box, and submit the exemption document for review. Please note, if your documentation does not include all requirements, it will be declined.
If you are lacking any of the requirements above, the following locations may be able to administer the immunizations or titers you need:
- The student health center at your home university/college
- Your doctor’s office or an urgent/convenience care center like MD360
- Flu Shots and PPD/TB tests are available at CVS, Walgreens and other family pharmacy/drug stores
- Titers are available at commercial labs like Any Lab Test Now or Labcorp

Typical immunization/test costs (for comparison only, prices are subject to change)
- Tuberculosis (TB) Testing
- 1-Step TB PPD: $25-$28
- 2-Step TB PPD: $50-$56
- QuantiFERON Gold blood test: $80 + $4 venipuncture fee
- Measles, Mumps & Rubella (MMR) Vaccination: $62-$73 + $25 administration fee
- Hepatitis B Vaccination (2-dose Heplisav): $115 + $25 administration fee per vaccination
- Varicella Vaccination: $113-$124 + $25 administration fee
- Flu Vaccination: $33-$35
- Titers: There is a $4 venipuncture fee for all lab draws, but you will only be charged once if multiple titers are done on the same day.

Clinical Instructors
These items are required only for clinical instructors who will accompany students on rotations in the clinical learning environment.
- Enter the expiration date (2 years from issue date) of your current BLS/CPR certification in the “New Value” box.
- Upload your current BLS/CPR certification document in the “Supporting Documents” section.
Please note: Prisma Health only accepts BLS/CPR certification from programs that follow the guidelines defined by the American Heart Association (AHA) or military training network. Your certificate/card must state that its training follows the guidelines of the AHA or was through the military training network.
Clinical instructors for nursing and nursing-related programs are required to complete additional program-specific items for clearance. Please view the information at the link below.
Clinical Nursing & Nursing-Related Instructors
Once you have entered and uploaded all required information into your checklist, a member of Student Affairs Administration will review it for approval by signing off in the “SAA Approval” section.
Note: In your compliance checklist, you may see the following icons:
- Green thumb up
- A green thumb up indicates the item has been approved and is clear.
- Red thumb down
- A red thumb down can mean two things:
- You have not yet submitted the item for approval
- The item is out of compliance
- A red thumb down can mean two things:
- Orange triangle
- The item will expire during your rotation period.
- This item is not out of compliance until the renewal date listed on your compliance checklist.
- This item is not affecting your overall clearance.
- Common items affected include TB, Flu, TDAP Respiratory Fit test, BLS/ACLS
- The item will expire during your rotation period.
Returning Faculty, Staff and Scholars
The following items must be renewed:
- You are required to submit a current flu season vaccination by November 1 each year.
- A current certification must be on file. BLS certifications are valid for 2 years.
- A valid nursing license must be on file.
- Learning Hub annual compliance training modules must be completed each year. New versions are released each summer.

The duration of this process depends on how quickly you are able to gather the necessary information and whether or not you have to obtain additional immunizations or tests.
Below, we have divided the clearance items by your role. If you are a clinical instructor rotating with students your clearance requirements will differ than someone performing research in an office building or offsite.
