00:15
my name is dr ashley jones i was the
00:17
critical care fellow in 2018 2019
00:20
and have now returned to my general
00:21
surgery residency as a chief resident
00:23
for my fourth and fifth year
00:25
i actually came here as a medical
00:27
student to do a visiting rotation
00:29
and i really was drawn to the trauma
00:32
service here
00:33
it's a busy trauma service i actually
00:35
did a
00:37
shadowing of the resident team and went
00:39
through a trauma activation
00:41
and really just enjoyed watching and the
00:44
residents
00:45
seemed to be really happy here and
00:47
seemed to be learning a lot and very
00:48
comfortable with what they did
00:50
from a critical care standpoint this
00:52
program is
00:53
really anything you want to make of it
00:55
we are a busy level one trauma center
00:57
we see a lot of penetrating trauma
00:58
there's a lot of operative experience
01:00
here
01:01
if you come from a place where there
01:03
wasn't a lot of trauma or critical care
01:05
this is a great place to get your feet
01:07
wet you leave here knowing
01:09
how to take care of critically ill
01:10
patients i'm chris watson i'm the
01:13
program director for the prisma health
01:15
surgical critical care fellowship
01:16
i trained here as a resident and left to
01:19
do my fellowship at the university of
01:21
virginia in charlottesville
01:22
and came back and two or three years
01:24
later took over the program there's a
01:25
couple of things that make this program
01:27
unique
01:28
we actually design the program around
01:30
what the fellow needs
01:31
we've noticed that not every resident is
01:33
the same all the training programs are a
01:35
little different
01:36
in order to give the fellow what they
01:38
really need
01:39
we have an interview after the interview
01:42
when they've already matched with us
01:43
when we decide what the program needs to
01:45
look like for the year
01:46
we have only one fellow and so you'll
01:49
really get to know our faculty
01:51
you'll work really close with those and
01:53
really develop a mentorship
01:54
with these people we have it so that the
01:56
residents and the fellows
01:58
really do have different roles here and
02:00
so there won't be a lot of competition
02:01
for cases or for patient care
02:04
one of the things that we really wanted
02:06
to focus on was
02:07
collaborative education for the fellow
02:10
and while
02:10
all programs are required to have a desk
02:12
for the fellow we had our fellowship
02:14
desk placed in an area that has the
02:16
pharmacist the
02:18
dietitian and one of the faculty members
02:21
usually me
02:22
and so when the fellow's planning
02:23
patient care they can turn around and we
02:25
can all talk about it
02:26
at the same time and it helps reaffirm
02:28
and focus that thought
02:30
and the residence so they really latch
02:32
on to that information
02:34
i've really enjoyed you know being in
02:36
colombia there's
02:37
multiple restaurants here it's a very
02:40
up-and-coming area there's multiple
02:42
breweries
02:42
there's always something to do on the
02:44
weekends and then if you're trying to
02:46
get out of town
02:47
or an hour and a half from charleston an
02:49
hour from charlotte you can get to the
02:50
mountains very quickly
02:52
it just seemed to be in a great location
02:56
so i think the most inspiring thing that
02:58
i've encountered here
03:00
is how this hospital system embraces
03:03
the patients in the community the trauma
03:05
service rallies around
03:06
their patients and supports them through
03:08
the entire hospital stay
03:10
them and their families and to watch you
03:13
know a patient come back
03:14
after we thought they weren't going to
03:15
make it into the icu to
03:17
you know say hello to the nurses and the
03:19
care team has been something that's been
03:21
very inspirational to me
03:22
so one thing that we're proud of here is
03:25
that we've been able to keep
03:26
a lot of our fellows on this faculty of
03:28
the seven that we have
03:29
currently four trained here and decided
03:33
to stay here
03:33
they love the area they love the
03:36
hospital system
03:37
and we all are very close and develop
03:41
close relationships with each other
03:43
after completing this program a fellow
03:45
is going to be prepared to do
03:46
every aspect of surgical critical care
03:49
we start the resident off with a lot of
03:51
oversight and by the end of the year
03:53
they're
03:54
running the ice suit independently we're
03:56
still there
03:57
if they need us sitting in the corner
03:59
watching but
04:00
they are running the show at that point
04:02
so the day they step
04:03
out of their fellowship they're going to
04:06
step right into being an
04:07
attending the resident team we've always
04:10
had each other's backs
04:11
and we all get along you know we're a
04:13
big family
04:15
we're away from all of our families so
04:17
we've become each other's family
04:33
you
