IRB Templates
Please download our IRB templates from Forms and Templates within IRBNet. After logging into IRBNet, Forms and Templates can be located from the left-hand side menu. You will need to have added a "Prisma Health - Greenville, SC" affilation within your User Profile in order to access our ICF and Protocol templates.
